Stages of head and neck cancer
The staging of head and neck cancer helps determine the extent of the disease and guides treatment options. The most commonly used system is the TNM staging system, which considers three main factors:
T (Tumor): Refers to the size and extent of the primary tumor.
- TX: Primary tumor cannot be assessed.
- T0: No evidence of a primary tumor.
- T1-T4: Size and extent of the tumor increase, with higher numbers indicating larger tumors or more extensive local invasion.
N (Nodes): Refers to whether the cancer has spread to nearby lymph nodes.
- NX: Regional lymph nodes cannot be assessed.
- N0: No regional lymph node involvement.
- N1-N3: Indicates the number, size, and extent of lymph node involvement, with higher numbers representing more extensive disease.
M (Metastasis): Indicates whether the cancer has spread to distant parts of the body.
- M0: No distant metastasis.
- M1: Distant metastasis is present.
Stages
Based on the T, N, and M classifications, head and neck cancer is generally grouped into stages:
- Stage I: Small tumor (T1), no lymph node involvement (N0), no distant spread (M0).
- Stage II: Larger tumor (T2), no lymph node involvement (N0), no distant spread (M0).
- Stage III: Either a larger tumor (T3) or any size tumor with regional lymph node involvement (N1), no distant spread (M0).
- Stage IV: More advanced disease, which can include:
- Stage IVA: Tumor of any size (T4) with or without lymph node involvement (N1-N3), no distant spread (M0).
- Stage IVB: Any size tumor with extensive lymph node involvement (N3), no distant spread (M0).
- Stage IVC: Any size tumor with or without lymph node involvement (N0-N3), with distant metastasis (M1).
